Cortisone for knee pain failure

Why Cortisone Shots Keep Failing You (And What Nobody Explains)

Why Cortisone Shots Keep Failing You (And What Nobody Explains)

[00:00:00]

You went in for a cortisone shot, maybe your first one, and honestly, it worked The pain backed off. You get back to doing the things that you love, and for a while you felt like yourself again. Then three months later, well, it came back. So you went back, you know, you got another shot. It worked again, but maybe not quite as well, and maybe not quite as long.

And now here you are, either on your third or fourth shot or being told that you've maxed out and it's time to talk about surgery. And somewhere in the back of your mind, you're wondering, "Why does this keep happening to me? Why does something that works keep stopping working?" That's exactly what I [00:01:00] want to talk to you about today, because the answer to the question is something that most people are never told.

And once you understand it, you'll never look at your knee pain the same way again. First, I want you to know something important. If you've been through this cycle, you're not alone, not even close. Cortisone injections are one of the most commonly performed procedures in musculoskeletal medicine.

Millions of them are administered every single year for knees, shoulders, hips, backs. And the vast majority of patients who receive them are told roughly the same thing: "This will help with inflammation. Come back if it stops working." That's often the entire conversation. No explanation of what's actually happening inside the knee.

No discussion of why the inflammation keeps returning. No roadmap beyond, "Well, we'll do it again," or, "Let's talk about [00:02:00] surgery." And here's what I find remarkable about all of this, is that we accept this as medicine in a way that we never would accept in any other area of our life. Imagine taking your car to your mechanic because the check engine light keeps coming on.

And every single time you go in, they just reset the light and send you home, and you keep paying for it. And the light keeps coming on. And eventually they just say, "Well, it might be just time for a new car." You'd find a new mechanic. But with our bodies, somehow we just keep going back for the reset. So let's talk about why and what's actually going on.

And let me be clear about something before I go further. I'm not here to tell you that cortisone is evil or that your doctor is wrong to offer it. It's not that type of conversation. Cortisone, or more accurately, corticosteroids, it's a powerful anti-inflammatory [00:03:00] agent. When injected into the inflamed joint, it essentially suppresses the inflammatory response in that area quickly, effectively, and for many people, dramatically And in certain situations like acute injuries, uh, pre-event pain management, buying time before a procedure, it has a legitimate role, but here's where the conversation usually stops and where it absolutely should not.

Cortisone does not restore cartilage. Cortisone does not address why the knee became inflamed in the first place. It is at its very core a very sophisticated way of turning down the volume on a very important signal in your body. Think of it this way. Inflammation in a joint is your body's alarm system.

It's loud, it's uncomfortable, it's disruptive, but there's a reason for it. It's your [00:04:00] body saying something needs attention here. Cortisone is like pulling the battery out of the smoke detector, right? The smoke is still there, the fire's still there, the house is still burning. It's just quieter now. And quiet doesn't mean resolved.

Now, here's where it gets really important, and this is the part almost nobody explains. Not only does cortisone fail to address the root cause of inflammation, repeated cortisone injections over time can actually accelerate the very degeneration you're trying to stop. I want to say that again because it's significant.

The most common treatment used for chronic knee pain when used repeatedly has been shown in research to potentially worsen the condition of the cartilage and the surrounding tissue. Here's why. Cartilage, that cushioning tissue between your knees, is already a challenging tissue to maintain. It [00:05:00] has very limited blood supply.

It depends on a very specific biological environment to stay healthy. The right nutrients, the right cellular signals, the right balance of breakdown and repair. Cortisone, when injected repeatedly, can interfere with that environment. It can inhibit the cells responsible for maintaining and repairing the cartilage.

It can weaken the surrounding tendons and ligaments, and over time it can change the biological terrain of the knee in a direction that makes genuinely healing harder, not easier. So here's the picture that nobody paints for you. You get a shot, the inflammation quiets down, the pain reduces, you feel better.

But the underlying problem, whatever is driving the inflammation, is still there. It's unchanged and now progressing. Then months later, the inflammation is back because the cause is still there, except now the tissue has had three [00:06:00] months, maybe more, of unaddressed degeneration and possibly some impact from the cortisone itself.

You get another shot, repeat, repeat, repeat. Each cycle, you're borrowing time, but paying a biological interest rate that compounds. Think about it like a credit card. The first purchase feels great. The payment feels manageable. But if you never pay down the principal, if you only ever pay the minimum payment, the debt grows, and eventually the bill comes due in a way that's much harder to manage than if you addressed it earlier. That's what's happening in your knee in this cycle. The cortisone, the minimum payment, the root cause is the principal, and nobody is talking to you about the principal. Now, I want to be fair here because I think it's important. Most doctors who offer cortisone injections are not doing it as something nefarious, right?

They're working within a [00:07:00] system, a system that is fundamentally designed around managing symptoms rather than resolving causes, a system where a 15-minute appointment, it doesn't allow for a deeper investigation, a system where the tools available within that model are largely responsible and procedural.

Cortisone fits the model. It's fast, it's measurable, it's billable, and it works in the short term. So it gets recommended. But here's the missing conversation, the one that almost never happens. Why is this knee inflamed? Not how do we turn down the inflammation, but why is it there in the first place? What is driving it?

Is it a mechanical problem, a metabolic issue, a circulation problem, a tissue quality issue, or some combination? Because here's the thing. Inflammation in a chronic knee is not a random event. It is a [00:08:00] response. Your body is responding to something, and that something has a cause. And chronic knee pain, the inflammation is often telling you that the tissue is breaking down faster than it can repair itself, that the biological environment inside the knee, inside that joint, has shifted in a direction that favors degeneration over regeneration.

That the knee is, in a very real sense, losing a battle it didn't have to lose. Cortisone quiets the battle cry. It doesn't win the war. And the question that should be asked every single time a cortisone shot is recommended for chronic knee pain, the question is, what are we doing to actually address why this knee keeps becoming inflamed?

If you've never been asked that question or offered an answer to it, that's not a reflection of your knee. That's a reflection of the model being used to treat it. So let me offer you a different way of thinking about your knee, one that I think is more honest, more [00:09:00] hopeful, and ultimately more useful.

Your knee is not just a mechanical structure that wears out like a machine part. It is a living tissue. And living tissue, given the right biological environment and the right inputs, has a capacity to respond, adapt, and in many cases, recover in ways that the conventional model doesn't give it credit for.

The question isn't just how do we manage the pain in this knee. The question is, what does this knee need to shift from a degenerative environment to a regenerative one? What needs to change about the way it's being loaded? What needs to change about the circulation to the tissue? What cellular repair processes aren't functioning the way they should, and can they be reactivated?

What is driving the chronic inflammatory cycle, and can that driver be addressed at the source? These are completely different questions, and they lead to completely [00:10:00] different answers. Now, I'm not gonna stand here and tell you that every knee is the same or that there's a magic solution. That would be dishonest.

And frankly, that kind of thinking is part of what got us in this mess in the first place. But what I'm going to tell you is this: the knee you've been told is just arthritic or just bone on bone or needs to be replaced may have been evaluated through a lens that wasn't looking for what could help it heal, only for what the next management step should be, and that's a conversation worth having.

So here's a takeaway from today. Cortisone shots fail not because they're poorly administered, and not because your knee is hopeless, but because they are designed to address the symptom, not the source. They borrow time that runs out. They quiet signals when we should be paying attention. And when used repeatedly, they can contribute [00:11:00] to the very degeneration they're meant to delay.

The real question, the one that changes everything, is why is the knee becoming inflamed in the first place? And that question requires a completely different answer, a completely different investigation, and completely different type of approach. In the next video, I'm gonna go deeper on what's actually happening inside a chronically damaged knee at the tissue level, at the cellular level, and why the conventional model misses it almost completely.

Because once you understand what's actually going on inside that joint, the idea that cortisone shot was going to fix it starts to become- starts to seem a little bit weak. A little bit like trying to fix a flat tire by turning up the radio so you can just stop hearing the thumping, right? If this made you think differently about your knee or about [00:12:00] someone you know that may be going through this, well, share it.

These are conversations that need to be happening, right? Subscribe if you want to follow this series. We're just getting started. I'm Dr. Bob Millham at Wake Integrative Health, and this is what root cause care actually looks like.
